Claiming your health fund rebate for contact lenses

Information For Health Fund Patients

EyeQ Optometrists practices are providers to all health funds and are fitted with HiCaps machines, which means you can claim your rebate on the spot.

In all online contact lens stores, including ours, things need to be done a little differently. You’ll need to claim your rebate back directly with your health fund after making your purchase with us.

We have worked closely with health funds to receive provider status for the online store, and these provider numbers can be found on the Health funds drop down menu during the checkout process. Simply select the health fund you’re a member of, and this health fund name as well as the provider number for the EyeQ Optometrists online store will then appear on the order receipt you receive by email after your purchase.

After your purchase simply apply for your rebate directly with your health fund in person, over the phone or online. You’ll need the order receipt as this shows the amount you spent, our provider number and the item code for your purchase.
